Partnership Form

Do I have to complete partnership and workforce forms as I am only a single organisation?

I am a franchise operation do I need to complete these forms?

All organisations, including franchise operations, wishing to claim from the UKHCA Skills for Care project, assuming we are successful, have to complete a partnership form. If these forms are not completed and returned to UKHCA we are unable to include your organisation in our bid and therefore if we are successful you will be unable to claim any funding.

I am part of a larger group of organisations operating within this company. Is it sufficient for the parent company to complete one partnership form and workforce development plan?

Skills for Care will accept the parent company completing a partnership form and workforce form on behalf of all their organisations. The parent company must return with the forms a list of the organisations covered by this one bid.

Contact Name. What do I enter in this part of the form?

The person who can answer questions on the workforce plan.

How do we complete the rest of the workforce plan form?

This is the number of units for each qualification multiplied by the number of workers wishing to undertake this qualification.

10 Care Assistants wishing to do NVQ2 @ 6 units per award = 60 units

1 Care Worker wishing to undertake NVQ3 @ 8 units per award = 8 units

2 Managers wishing to undertake NVQ4 @ 8 units per award = 16 units

Inductions 1 unit each

Foundation 1 unit each

Verifier 1 unit old and new qualifications

Assessor 1 unit old and new qualifications

Units to be funded under Skills for Care England 2006/07

If the organisation does not have any other source of funding for this training then the answer is the same as Total Units Required. In some instances the organisation has some funding from elsewhere if this is the case this should be entered into “Funding from other Sources”.

Skills for Care is not able to fund all your training requirements. It is assumed that you will obtain funding from other sources.

The two columns should add up to Total Units Required.
